YOUR FIRST VISIT

When you arrive for your first visit with Lois Lagier, DDS, please be prepared to complete all new patient forms below that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.

We take pride in making your dental experience as pleasant and comfortable as possible. We will thoroughly present treatment options, fully explain our procedures and provide customized instructions for you to maintain your oral health. It is our goal to have you feel secure in the treatment of your dental needs.

If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them on your arrival.
